Recommend a Long-Acting Nitrate for Angina Based on Ease of Use and Cost

We're getting questions about how to optimize therapy for angina...when the maximum tolerated dose of a beta-blocker isn't enough.

Suggest a calcium channel blocker. Recommend SWITCHING from a beta-blocker to verapamil or diltiazem...if a beta-blocker isn't tolerated.

Or recommend ADDING a dihydropyridine CCB (amlodipine, etc) to a beta-blocker if needed to control angina or BP...but watch for edema.
